If the applicant applied for a single kitten, do they have another playful cat at home to ask as a companion for the adopted kitten? If the answer is *no*, please immediately contact the applicant to explain that the Cat Shack does not adopt single kittens as only-pets (you can forward the "kittens in pair" info sheet) and a pair of kittens should be suggested, or recommend that they consider a 1 year old or older cat if they are only interested in an only-pet.* Choose one: Yes No

Did the Vet Office indicate that the applicant treats their pets when they are sick? (diagnostic tests, specialists, admit them to the vet for care, treat cats with chronic illnesses such as diabetes, etc)*
If there are questions about the vet reference, contact the application to explain the issues and ask if there are other veterinarian offices that the application has used who could fill in the gaps in the reference.
